General Information on Ordering Utah Death Certificates
In Utah, the civil registration of births, deaths, and marriages
developed slowly, culminating with a statewide system of recording
births and deaths beginning in 1905. Early legislative acts empowered,
but did not require, certain cities to maintain a register of births
and deaths. In 1898, the state legislature provided for central
county records, requiring county clerks to keep separate birth and
death registers. Since marriage was seen as a religious sacrament,
the civil registration of marriages was not required in Utah until
1887.
Certificates for deaths that occurred in Utah since 1905 are on
file with the Utah Department of Heath, Office of Vital Statistics.

State Archives.
Death certificate fees: Search fee (includes one certified copy)
- $13.00 Each additional copy of the same record - $8.00
NOTE Identification is now required for the purchase of a Utah
death certificate. Mailed requests must include an enlarged and
easily identifiable photocopy of the back and front of your identification
from the list below. If no proofs are enclosed, your application
will be returned.
