General Information on Ordering Pennsylvania Death Certificates

The Division of Vital Records issues only certified copies of death records with the raised seal. Death records are available from 1906 to the present. For death records prior to 1906, refer to the Pennsylvania State Archives.

Eligible Persons Who May Request a Death Record:

  • Legal representative of decedent's estate
  • Immediate family members
  • Extended family members who indicate a direct relationship to the decedent

All requests must be submitted on an application form and include the signature and valid government issued photo ID of the individual requesting the record. Examples of acceptable identification are a state-issued driver's license or non-driver photo ID with requestor's current address, passport, etc. If possible, photo ID should be enlarged on copier by at least 150%.
