General Information on Ordering Ohio Death Certificates
Ohio statewide vital records registration began in 1908. Two previous
attempts to enact such legislation were largely unsuccessful, though
fragments exist.
The Ohio Department of Health houses only the death records of
people who died in Ohio from Jan. 1, 1945, to the present. If the
death did not occur in Ohio, contact the Vital Statistics office
in the state where it occurred.
Death records that occurred December 20, 1908-December 31, 1944,
can be obtained from the Ohio
State Archives, 1982 Velma Avenue, Columbus, OH 43211-2497.
