General Information on Ordering Indiana Death Certificates
Certificates for births or deaths can be obtained from the Local
Health Department in the county where the event occurred or by mail
or fax from the Indiana State Department of Health. All requests
require proper identification. Methods of payment and fees may vary.

Death registration in Indiana began in 1882. Prior to 1900, records
of death are filed ONLY with the Local Health Department in the
county where the death actually occurred. There are no indexes to
the state death certificates from 1900 through 1918. The specific
CITY or COUNTY of death must be provided for searches in this period
and the search is limited to one county for each Search Fee ($8.00)
paid. If the exact date of death is not known, please indicate the
five (5) year period to be searched. An additional $8.00 is required
for each additional five (5) year period searched, or COUNTY SEARCHED
from 1900-1918. Search fees are nonrefundable.
Individuals seeking birth and death certificates can apply in person
at the State Vital Records Office from 10 a.m. to 2 p.m., weekdays.
The office is located at 6 Washington Street in downtown Indianapolis.
