General Information on Ordering Florida Death Certificates

Please provide a signed letter with the following information. Applications are accepted by mail, fax, phone or in person at the State Vital Statistics office in Jacksonville.

• Anyone may order a Florida death certificate

• Limited Florida Deaths were recorded between the years of 1877 and 1917.

• Cause of death becomes public information 50 years following the death. Prior to that time, cause of death is considered confidential by Florida law.

• Provide information about the person named on the certificate

• Provide information about the person ordering the certificate

• Select type of death certificate: (photocopy certification, additional copies)
